Tongue Swelling or Lump: Causes and When to See an ENT Specialist

What Does Tongue Swelling Feel Like?

Patients may notice:

  • Enlarged or thickened tongue
  • A firm lump or ulcer on the tongue
  • Discomfort or pain when moving the tongue
  • Difficulty speaking or chewing
  • Altered taste sensation
  • Bleeding or numbness in advanced cases

Tongue swelling may be painless initially, which delays medical review.


Common Causes of Tongue Swelling

1. Infection or Inflammation

Bacterial or viral infections can cause acute painful swelling.


2. Trauma or Biting Injury

Usually painful but resolves within days.


3. Allergic Reaction (Angioedema)

Rapid swelling, often sudden, may threaten the airway.


4. Benign Tongue Lesions

Such as cysts or vascular lesions.


5. Tongue (Oral) Cancer

🚨 Red Flags You Must Not Ignore

Seek urgent ENT assessment if tongue swelling:

  • Persists more than 2 weeks
  • Is firm, hard, or progressively enlarging
  • Is associated with bleeding or ulceration
  • Causes difficulty speaking or swallowing
  • Is accompanied by neck swelling
  • Occurs in smokers or heavy alcohol users

Persistent tongue swelling in adults is cancer until proven otherwise.
