
Today, I am going to share an article from the Laryngoscope Journal about a feminization procedure known as chondrolaryngoplasty, which is a surgery aimed at removing the Adam’s apple. This is a cosmetic procedure that is rarely performed in Malaysia. However, for the sake of knowledge, let’s explore the article (the link is provided below)

Chondrolaryngoplasty is a procedure designed to reduce a conspicuous laryngeal prominence and feminize the neck. It has applications as a gender-affirming surgery that should be offered to assigned male at birth patients undergoing management of gender dysphoria.
A prominent thyroid notch is due to the developmental impact of testosterone on the laryngeal cartilages and cannot be reversed during estrogen therapy. The laryngeal prominence is most pronounced superiorly, whereas the attachments of the laryngeal muscles, epiglottis, and vocal folds are all more lateral or inferior, allowing for safe removal of the more prominent superior cartilage.
Traditionally, this procedure is performed through a transcervical incision, which may lead to an unsightly visible scar that can “out” the patient. To address this, indirect approaches have been employed utilizing a submental incision, which can be concealed by the chin. This approach nevertheless has the result of a visible neck scar and carries the risk of tethering to the underlying cartilage framework. To address this issue, Khafif et al. described scarless chondrolaryngoplasty through a transoral endoscopic vestibular approach (TOEVA).
This group published the first and only series of this approach performed on four patients with excellent outcomes. They identified the anterior commissure using transillumination of a flexible laryngoscope, used intermittently throughout the procedure.
