A blocked nose that lasts for weeks is rarely just a cold. It is one of the most common complaints I hear in clinic. If you are always reaching for tissues, snoring loudly or sleeping with your mouth open, something else is going on inside your nose.
The usual suspects are allergic rhinitis, a deviated septum, enlarged turbinates, nasal polyps and chronic sinusitis. Allergies cause swelling inside the nose. A bent septum or polyps physically block airflow. Many patients have a combination of these, which is why guessing and treating yourself rarely works.

Here is what the evidence says. Cochrane reviews, the gold standard of medical research, confirm that intranasal corticosteroid sprays significantly reduce blockage from allergic rhinitis and nasal polyps, and are safe long term when used correctly. However, no spray can straighten a deviated septum. Structural problems sometimes need a simple day surgery (septoplasty).

Also, do not rely on pharmacy decongestant sprays for more than a few days. They treat the symptom, not the cause. Overuse causes rebound swelling and makes the blockage worse (Rhinitis medicamentosa).
