The surgery requires a small incision placed within the natural skin creases of the neck. I carefully dissect the cyst away from surrounding vital structures, including nerves and blood vessels, ensuring the entire cyst wall is removed to prevent recurrence. Indications for Surgery

I recommend surgical excision when a cyst poses a risk to health or significantly affects quality of life. The strongest clinical reasons include:

  • Recurrent infections: Cysts that repeatedly become infected, swell, or form painful abscesses requiring emergency drainage.
  • Progressive enlargement: Cysts that grow large enough to cause visible cosmetic deformity or compress nearby structures like the airway or swallowing pathway.
  • Risk of malignant transformation: Thyroglossal duct cysts carry a 1 % risk of developing into malignancy, most commonly papillary thyroid carcinoma. Surgical excision eliminates this risk and allows for definitive histopathological evaluation.
  • Diagnostic uncertainty: Neck lumps where malignancy cannot be entirely ruled out without complete surgical excision and histopathological analysis.
  • Chronic discharge: Cysts that form fistulas and continuously leak fluid onto the skin, causing irritation and social discomfort.
  • Failure of medical management: Cysts that do not resolve or continue to enlarge despite appropriate courses of antibiotics or anti-inflammatory medications.

Complications of the Surgery

  • Wound infection: 2 to 5 percent. Managed effectively with a short course of oral or intravenous antibiotics.
  • Haematoma: 1 to 3 percent. A collection of blood under the skin that may require minor drainage to relieve pressure.
  • Nerve injury: 1 to 2 percent. Temporary or permanent weakness of the marginal mandibular nerve, affecting lower lip movement, or the hypoglossal nerve, affecting tongue movement, depending on the cyst location.
  • Cyst recurrence: 2 to 5 percent. This occurs if microscopic remnants of the cyst lining are left behind during surgery.
  • Keloid or hypertrophic scarring: 1 to 3 percent. Raised, thickened scars that are more common in certain skin types, though I use meticulous closure techniques to minimize this risk.
  • Salivary fistula: Less than 1 percent. Leakage of saliva if the cyst is closely attached to the parotid or submandibular gland.
