Indications for Surgery
I do not suggest surgery for every swollen sinus. We only proceed when the physical blockage directly ruins your quality of life and medical therapy has failed. The strongest clinical reasons include:
- Chronic rhinosinusitis failing maximal medical therapy: Persistent inflammation lasting over twelve weeks despite consistent use of steroid nasal sprays and antibiotics.
- Recurrent acute rhinosinusitis: Four or more severe bacterial sinus infections within a single year.
- Chronic rhinosinusitis with nasal polyps: Benign growths inside the nasal cavity causing complete airway blockage and loss of smell.
- Fungal sinusitis: Fungal ball or allergic fungal inflammation requiring mechanical removal.
- Sinus mucoceles: Mucus filled cysts expanding within the sinus cavity and eroding surrounding bone.
- Orbital decompression: Creating space behind the eye for patients with severe thyroid eye disease.

Risks and Complications
Every surgery carries inherent risks. I discuss these thoroughly during our consultation so you make an informed decision. The overall complication rate for FESS is very low when performed by a specialist.
- Bleeding (epistaxis): 1 to 5 percent. Minor oozing is normal for a few days. Severe bleeding requiring medical intervention is rare.
- Infection: 1 to 2 percent. We prescribe prophylactic antibiotics and recommend saline rinses to keep this risk minimal.
- Adhesion formation (synechiae): 1 to 3 percent. Scar tissue can form between the nasal septum and the lateral wall. We use careful technique and temporary splints to prevent this.
- Orbital injury: Less than 1 percent. The thin bone separating the sinuses from the eye can fracture. This might cause bruising or double vision. Permanent vision loss is exceedingly rare.
- Cerebrospinal fluid (CSF) leak: Less than 0.5 percent. The thin barrier between the nasal cavity and the brain can be breached. We repair this immediately if it occurs.
- Nasolacrimal duct injury: Less than 1 percent. The tear duct can be damaged during surgery near the front of the sinus, causing excessive tearing.
